VSBA Meeting Registration Form

Cancellation is required 48 hours prior to the event. Cancellation within 72 hours and/or failure to appear will result in full payment for registration fees. VSBA has the right to cancel a workshop if the registration has insufficient quantities.

Please list a daytime contact phone number for the registrant in case of late cancellation due to weather or other uncontrollable conditions.
